Healthix, a health information exchange serving New York City and Long Island, has partnered with Unite Us, a health IT vendor, to enhance the interoperability of social determinants of health (SDOH). The collaboration aims to establish a seamless infrastructure for communication between healthcare systems and social care networks, enabling better access to resources such as food, housing, and transportation assistance. This integration of health and social care seeks to improve health equity among underserved populations in New York.
Healthix, a health information exchange (HIE) serving New York City and Long Island, has joined forces with health IT vendor Unite Us to advance the interoperability of social determinants of health (SDOH).
This partnership aims to establish a robust infrastructure that facilitates communication between healthcare systems and social care networks, with a focus on addressing needs related to food, benefits, housing, and transportation assistance.
By integrating health and social care, the initiative aims to enhance health equity among underserved populations in New York. Research indicates that only 20 percent of health disparities within a population can be attributed to medical care, while the remaining 80 percent are connected to SDOH.
Todd Rogow, President and CEO at Healthix, expressed enthusiasm about collaborating with Unite Us, stating, “We welcome the opportunity to partner with Unite Us to deliver a comprehensive level of care to our customers, both within and beyond hospital walls.” Rogow believes that the Unite Us software will create a seamless and user-friendly way for customers to access a network of healthcare and social care providers who can connect them with available community-based resources, such as housing, employment, and behavioral health assistance.

He highlighted their shared mission of improving the lives of individuals in NYC and Long Island by supporting community-led systems of care, building trust within these communities, and capturing a comprehensive understanding of a person’s health.
Unite Us and Healthix have previously collaborated in large social care networks with Public Health Solutions, an organization that provides resources to underserved populations in NYC.

The technologies offered by Healthix and Unite Us support interoperability, critical care management, closed-loop referrals, and population health capabilities.
